For Referring Providers

Our Values and Partnership

We view our team as an extension of the referent, understanding that they hold their clients in high regard. We consider our referents as collaborators and an integral part of our clients’ recovery journey; always remembering they are our referents client first. Our goal is to enhance and continue the quality care that they have already been provided. We maintain regular communication with referring professionals, keeping them informed about the client’s progress at every stage.

Our approach balances compassion with accountability, addressing the complex needs of high-functioning professionals and individuals of means. By partnering with us, professionals can be assured that their clients will receive exceptional, integrative, and personalized care.

We understand the importance of finding the right program when the client needs a higher level of care. Our mutual goal is to offer each individual the best possible chance to achieve long term recovery and restore balance to their lives. We are committed to providing the highest quality of individualized care in a nurturing environment.

Healthcare Provider
(Non-Recovery)

Doctors play a vital role in identifying substance use disorders, even if addiction isn’t their expertise. Recognizing the signs of addiction is often the first step, but many may not know how to proceed.

1. Engage

We help doctors identify and address addiction by creating a supportive setting for patient conversations about substance use. Our approach features a safe environment for open dialogue with non-threatening questions like, "How many times in the past year have you used a prescription medication or illegal drug for nonmedical reasons?" or “Has your work or performance ever suffered due to drinking or drug use?”.

2. Identify

Detecting substance use disorders and identifying drug-seeking behavior can be challenging. Physicians should be attentive to signs such as inconsistencies in patient symptoms, impatience, and obsessive behavior. We provide support to healthcare providers who may not specialize in addiction, assisting them in recognizing physical, behavioral, and psychological warning signs of substance use disorder. Thorough physical examinations and targeted tests are essential for distinguishing between drug use for genuine physical pain and drug abuse behaviors. Discrepancies in exam responses and urine toxicology results may signal potential substance misuse.

3. Educate

Understanding the complexities of substance use disorders is crucial for effectively supporting patients. Given that addiction can impact various aspects of a patient's health, it is essential for healthcare providers to have a foundational knowledge of the disease. We must collectively understand that addiction overrides personal choice and willpower. Refining our understanding of addiction allows us to target and help those who may not fit traditional stereotypes of an alcoholic or a drug addict. Early detection is key as it can prevent the progression of substance use disorders, enhance treatment options, reduce the likelihood of comorbid conditions, and lower the risk of potentially life-threatening situations.

4. Collaborate

Our team of trained and experienced professionals is equipped to intervene at any stage of addiction and collaborate with the referring healthcare provider. Engaging the entire care team allows for a comprehensive and accurate assessment, leading to more informed and effective treatment strategies. Using advanced techniques such as motivational interviewing to encourage behavior modification, our experts are adept at guiding clients toward positive change. Using feedback from both the client and the referent guides clinical decisions, ensuring steady progress and the most comprehensive care.

5. Implement

We are committed to seamlessly integrate recovery strategies in the client's established goals and plans, in collaboration with healthcare providers they currently work with. Our approach emphasizes the importance of incorporating non-pharmacological treatments, such as behavior modification modalities, before considering the prescription of medications. By encouraging clients to explore these alternative therapies, we aim to enhance their engagement. Patients who are genuinely seeking relief are more likely to benefit from and adhere to these non-pharmacological approaches. Together with referring providers, we ensure that these strategies are effectively implemented, tailored to individual needs, and aligned with overall treatment objectives, facilitating a comprehensive and supportive path to recovery.

While many physicians do not specialize in recovery, they can still play a vital role in identifying and managing substance use disorders. By being able to recognize the signs of addiction and gaining a foundational understanding of misuse and abuse, physicians can significantly improve client outcomes and help mitigate the risk of addiction and relapse. It is crucial for all physicians to receive relevant education and remain open to collaborating with recovery professionals. This collaborative approach ensures a well-rounded strategy for client care, benefiting the overall health and long-term recovery of the client.

We find that ongoing communication along with accountability monitoring allows us to keep a finger on the pulse of their progress and implement early course correction when needed. This can prevent returning to unhealthy behaviors and mitigate the need for recurrent high-intensity inpatient treatment.

Recovery Professionals

Our approach to working with recovery professionals is designed to complement and enhance the care already provided, ensuring a seamless integration of support for clients.

1. Develop a Multidisciplinary Recovery Plan

We collaborate closely with the referring professional to create a comprehensive recovery plan. This plan integrates the expertise of all involved parties, including the referring provider, to address the client's needs holistically. Our role is to support and augment the existing strategies, providing high-touch support as needed.

2. Provide Additional Support

When a referring professional encounters challenges such as difficulty in implementing lifestyle changes or managing a particularly needy client, we step in to provide the necessary support. This can include in-home assistance or additional resources to help the client adhere to recommendations. We offer this high-touch support to alleviate the burden on the referring professional and enhance client engagement. We find that ongoing communication along with accountability monitoring allows us to keep a finger on the pulse of their progress and implement earlier course correction when needed. This can prevent returning to unhealthy behaviors and mitigate the need for recurrent high-intensity inpatient treatment.

3. Stabilize During Crisis

In cases where a client is in crisis and requires immediate intervention, our team is prepared to provide urgent support. Whether the client needs immediate behavioral intervention or comprehensive care from a full recovery team, we are equipped to respond swiftly and effectively, working alongside the referring professional to stabilize the situation.

4. Supplement Existing Care

For clients with an established care team, our role is to provide additional support where needed. Our approach involves working in tandem with the existing team to help achieve the predetermined goals, making sure our involvement supports rather than substitutes the referring professional’s role.

5. Provide Higher Intensity Care

Clients may not follow a straight path in their treatment journey, sometimes requiring a shift to a higher level of care. For example, a client who has been in recovery for three months with regular therapy sessions might relapse and need more intensive support than a therapist can provide. As clients move through various stages of addiction treatment, their needs can change significantly, reflecting the concept of a “continuum of care.” We are equipped to provide a higher intensity of care that many referring professionals may not be able to offer alone. Our services include advanced, medically and clinically managed addiction treatment as well as integrated wellness services, designed to address these evolving needs. We work collaboratively with referring professionals to assess the most appropriate level of care at every stage for each client, ensuring that they receive the comprehensive support required for their recovery journey.

1. Personalized Introduction and Assessment

Upon referral, clients will be introduced to our dedicated Recovery Care Coordinator, who will serve as their primary point of contact. They will then meet with our addiction medicine physician, Dr. Karen Flannery, to begin a comprehensive assessment. This assessment includes evaluating the client’s medical history, co-occurring disorders, previous recovery attempts, physical and mental health, social and familial dynamics, employment status, and motivational factors.

2. Tailored Treatment Plans

Our team will collaborate with the referring treatment center staff to develop a personalized, in-home transitional care plan that addresses the client’s specific needs and goals. This plan is crafted with input from the client’s existing support system, which may include family, friends, therapists, or recovery coaches. Our objective is to integrate our services seamlessly into the client’s lifestyle, leveraging their current support network to create a cohesive and effective recovery strategy. Each plan is tailored to fit the client’s unique requirements, ensuring the smooth and effective incorporation of personalized recovery strategies into daily life. We find that ongoing communication along with accountability monitoring allows us to keep a finger on the pulse of their progress and implement early course correction when needed, with the goal of preventing their return to unhealthy behaviors and mitigating the need for recurrent high-intensity inpatient treatment.

3. Seamless Integration

We begin preparing for each client’s transition from residential treatment to their home as early as possible. Our goal is to ensure a smooth and seamless transition, achieved by closely coordinating with the referring treatment center well before the client’s discharge date. Our team works collaboratively with all involved parties to implement the customized treatment plan, ensuring continuity of care. We execute the plan diligently, providing ongoing support, tracking progress, and addressing challenges every step of the way.

4. Ongoing Collaboration

Throughout the client’s recovery journey, we maintain open communication with the referring treatment center. This collaboration ensures that the treatment plan is adjusted as needed and that all parties are aligned in their efforts to support the client’s recovery. Regular updates and feedback are established to monitor progress and address any emerging issues promptly. By working together continuously, we ensure that the client receives consistent and comprehensive care, enhancing their chances for sustained recovery.
